  • A copy of last year’s W-2 form, if applicable
  • A copy of your most recent federal tax return, if you were self-employed before being disabled

You will also be required to fill out additional paperwork pertaining to your medical records and other requested information for the SSA.

It can take several months for your disability application to be processed, and unfortunately, since most disability applications are initially denied, you may have to file an appeal.
