In cases where an insurance company decides to deny your application for long-term disability benefits, it is required to send you a formal letter explaining the reason or reasons for that denial. The benefits denial letter from your insurance company should provide detailed information about all of the following:
If your benefits application has been denied, you might be eligible to appeal that decision. There is certain medical documentation which should be included as part of any appeal. You should include copies of all related medical and physical therapy records, imaging studies, and evaluations which your doctors have performed. It is also important that the medical documentation be very specific about what activities you are unable to do (including daily living tasks) and the specific limitations you are faced with as a result of your specific illness or injury.
You could also include documentation from family or friends which evidences their first-hand observations of your condition (as opposed to subjective opinions about your medical condition). For example, a statement that your spouse must assist you with going up and down stairs or with helping you get into the shower could be helpful to your case.
