One reason might be that you did not supply the proper documentation when you made your insurance claim or that you failed to follow some other protocol. Your Legal Options if you Have Been Denied ERISA Benefits

Prior to making a claim or filing a lawsuit directly against your insurance company, you first have to exhaust all of the administrative appeal remedies with your insurance company. Insurance companies are required to allow you at least one chance at an administrative appeal. 

The denial letter from your insurance company should set forth exactly what you need to do in order to file an administrative appeal in your case, and it should state the deadline for doing so. If you fail to take action by the deadline, your administrative appeal will likely be denied. 

Also, during the administrative appeal process, you might need to obtain additional medical records or other documentation. That additional documentation might include a letter from your doctor stating that in his or her opinion, you are suffering from a long-term illness or injury that prevents you from working.
