Typical Reasons for Life Insurance Benefits Claim Denials

There are several reasons why an insurance company may decide to deny a claim for life insurance benefits. Some of the most common reasons for life insurance benefit denials include the following:

  • The decedent’s death falls within a policy exclusion — Many insurance policies, including life insurance policies, have an exclusions section. This section sets forth various scenarios where a policyholder’s death may be excluded from coverage. In the context of life insurance policies, exclusions typically prohibit coverage in cases where the policyholder contributed to his or her death or committed suicide.
  • The beneficiary failed to make a timely claim — Insurance policies also typically set forth a timeline by which a claim must be made following the policyholder’s death. If the designated beneficiary does not make a claim within this time frame or otherwise fails to follow insurance company protocols, then the insurance company may decide to deny the claim.
  • Lapse of premium — During the lifetime of a life insurance policy, the insured is required to make regular premium payments at certain intervals. In the event the policyholder stops making his or her premium payments, and the policy lapses, then the insurance company may deny a benefits payout claim made by a designated beneficiary under the policy.

If the insurance company has denied your claim for life insurance benefits, there are several remedies that you may have. First, you may be able to file an administrative appeal with the insurance company in an attempt to obtain those benefits. At this stage, you can submit additional documentation in support of your claim.

After you have exhausted all of the administrative remedies, you may be able to file an appeal in federal court — especially if the plan is an employer-sponsored plan. In federal court, a judge makes the final decision in the case about an individual’s entitlement to benefits.
